Not everyone who contacts adult social care needs ongoing, long-term support. We may refer you to our Supporting Independence Team, which can offer short-term support to help you to stay independent.
You will also receive support from this team if you are an unpaid carer or have a sensory impairment.
Supporting independence
A Community Connector worker will develop a plan with you to help you identify the outcomes you want to achieve. They will also complete a supporting independence assessment to identify the best support for you. Your worker will discuss how long your support will last. Types of support might include:
- access to community services that help with isolation and loneliness
- linking you to practical support for your health and wellbeing
- providing assistive technology and equipment that will help you in your home
- help with housing issues
- access to advice about benefits and financial support
- access to health and wellbeing groups
- access to employment, training, or volunteering opportunities
