Before starting this form, you will need the following:
- Debit/credit card to make payment
- If you are making payment by PayPal please make sure you have your username and password before starting this form
To complete your application, you will need:
- Details of your insurance policy
- Details of the dimensions of any accommodation that you are planning on using to house the animal(s)
- Number of animals to be kept along with the sex of the animals
- Scientific name and common name of the animals
For security purposes, you may be required to verify your payment by logging in to your banking app or receiving an authorisation code via email and/or SMS. Please have something to hand which will allow you to verify it is you making the payment. Any delay in verifying will cause the form to timeout and may mean your payment is not successful.
