Before starting this form you will need the following:
- Debit/credit card to make payment
- If you are making payment by PayPal, please make sure you have your username and password before starting this form
For security purposes, you may be required to verify your payment by logging in to your banking app or receiving an authorisation code via email and/or SMS. Please have something to hand which will allow you to verify it is you making the payment. Any delay in verifying will cause the form to timeout and may mean your payment is not successful.
To complete your application, you will need:
- Your insurance details.
- Details of the animals in your care
- Details of any performances or encounters, including the time of the events & the duration.
- Emergency keyholders details
If you are renewing an existing licence, you may also need:
- The name of the authority where the licence was previously granted
