Before starting this form you will need the following:
- A debit or credit card to make payment
- If you are making payment by PayPal please make sure you have your username and password before starting this form
For security purposes, you may be required to verify your payment by logging in to your banking app or receiving an authorisation code via email and/or SMS. Please have something to hand which will allow you to verify it is you making the payment. Any delay in verifying will cause the form to timeout and may mean your payment is not successful.
If you are submitting a new application or renewing an existing application, you will require:
- The maximum number of animals can house.
- Details of the dimensions of any accommodation that you are planning on using to house the animal(s)
- Veterinary details
- Insurance details
If you are renewing your licence, you will also require:
- Your existing licence number
