Use this form to refer a child or young person to the Educational Psychology Service.
This form is for schools and settings only to complete.
Before starting this form, you will need:
- your school to have bought services from the Educational Psychologist Service (EPS) using Trade Rotherham.
- a completed consultation by one of our Educational Psychologists
- the child or young person’s completed Special Educational Needs Support Plan
- the parent or carers consent
- completed 'Views of the Parent or Carers' document
- completed 'Views of the Child or Young Person' document
The Views of the parent or Carer and Views of the Child or Young Person documents must be agreed and signed by the parent or carer or the young person if over 16 years of age and able to give informed consent.
By completing this form, you are confirming that the Parent or Carer consent has been provided and a signed copy of this agreement is held on the school file. Information leaflets are available to share with parents or carers and young people to help gain informed consent and can be downloaded.
