Sandygate Supported living
Our Aim
At Sandygate we will use person centred, strengths based and asset-based approaches to ensure that the intervention is about the individual and their life and circumstances. Following an individual assessment, a detailed support package will be developed to maximise strengths and opportunities to enable each service user to achieve their desired outcomes whilst meeting their needs and aspirations where possible.
Working in an individualised way will protect and build on the service user’s resilience, encourage choice, and improve wellbeing.

- Supported living provides services to adults with a learning disability for adults under 65 years and above 65 years
Sandygate Support living
Sandygate is a 3 bedroom bungalow that provides a support living service for adults with learning disabilities who have been individually assessed by a qualified social worker or other persons who may be qualified to carry out this assessment. The property is owned by Golden Lane Housing who work in partnership with Rotherham Borough Council.
Referrals into the service:
- Initial Referral
- Care Assessment
- Details of Diagnosis – if appropriate
- Other relevant information; Care Plans, Life History, Pen Picture, Risk Assessment, information from other services if appropriate
- It is important that all information is current, complete and up to date
Once a referral is accepted:
- Visit to Sandygate
- Tea Visits
- Gradual introductions and overnight stays
- Compatibility assessments to be undertaken
- Tenancy agreements and meetings with existing tenants and Golden Lane housing to agree terms and conditions
Services and Facilities
- All residents are provided a key for access to their home
- The service is designed to provide 24 hour staffing for individuals who require support in relation to physical and emotional needs. Staff are on call for emergencies during the night
- We aim to provide a safe environment where individuals can experience a greater level of independence whilst living in their own home, still having a safety net and support,when needed from staff
- The property provides a shared living facilities which includes, bathrooms and toilets, sitting room, kitchen, dining area, and garden
- The service provides one staff member on duty with two staff members at targetted times, if required, which includes sleeping duties
- Service agreements are in place alongside joint tenancy meetings
Ensuring Quality
Service users, carers and other people who are involved in the service will be consulted with regularly to enable the service to seek views and to use these to develop and improve service provision. The findings of the quality assurance, including any required action plan for improvement will be made available.
