Acres View Supported living
Our Aim
At Acres View we will use person centred, strengths based and asset-based approaches to ensure that the intervention is about the individual and their life and circumstances. Following an individual assessment, a detailed support package will be developed to maximise strengths and opportunities to enable each service user to achieve their desired outcomes whilst meeting their needs and aspirations where possible.
Working in an individualised way will protect and build on the service user’s resilience, encourage choice, and improve wellbeing.
Staff
A commitment is given to the training and continuous development of all its’ employees in order that they may acquire knowledge, skills, qualifications, and attitudes to performing their duties competently in their job role.
All staff are recruited through a rigorous vetting and approval process. This includes enhanced DBS, references, and health checks.

- Supported living provides services to adults with a learning disability for adults under 65 years and above 65 years.
Acres View Supported living
Acres view is a detached house with 4 bedrooms that provides supported living service for adults with learning disabilities who have been individually assessed by a qualified Social Worker or other persons who are deemed has been qualified to carry out this assessment. The property is owned by Golden Lane Housing who works in partnership with Rotherham Borough Council.
We provide the tenants at Acres view with support with housing ,social and personal care support to maintain their tenancy agreement which conforms to service specifications and legislative requirements within Rotherham Borough Council, and the Care Quality Commission for Social Care Inspection (CQC).
Referrals into the service
The following information will be required:
- Initial Referral
- Care Assessment
- Details of Diagnosis – if appropriate
- Other relevant information; Care Plans, Life History, Pen Picture, Risk Assessment, information from other services if appropriate
- It is important that all information is current, complete and up to date
Once a referral is accepted
The referral information will be assessed and a written contract will be made with the referrer. Careful consideration will be given to introductions – the use of:
- Visit to Acres view
- Tea Visits
- Gradual introductions and overnight stays
- Compatibility assessments to be undertaken
- Tenancy agreements and meetings with existing tenants and Golden Lane housing to agree terms and conditions
Services and Facilities
- All residents are provided a key for access to their home
- The service is designed to provide 24 hour staffing for individuals who require support in relation to physical and emotional needs. Staff are on call for emergencies during the night
- We aim to provide a safe environment where individuals can experience a greater level of independence whilst living in their own home, still having a safety net and support,when needed from staff
- The property provides a shared living facilities which includes, bathrooms and toilets, sitting room, kitchen, dining area, and garden
- The service provides one staff member on duty with two staff members at targetted time, if required, which includes sleeping duties
- Service agreements are in place alongside joint tenancy meetings
Ensuring Quality
Service users, carers and other people who are involved in the service will be consulted with regularly to enable the service to seek views and to use these to develop and improve service provision. The findings of the quality assurance, including any required action plan for improvement will be made available.
