What is a social care assessment?

A social care assessment is carried out by the Council to understand your care and support needs. We will gather information about you and your life to understand your strengths, any difficulties you have, and how your wellbeing is affected.

We can then make informed decisions about whether you're eligible for support.

How to get a social care assessment

If you would like to have a social care assessment, please contact us. Before you contact us on behalf of someone else, you should have their consent to do so where possible.

Most people end up paying for some or all of their own care.
