Care and support plan
We will work with you to develop a care and support plan. Your plan will show how your eligible needs will be met. We will support you to organise the right care and support services to achieve the goals in your plan.
Financial assessment
We will do a financial assessment with you to look at how much you may need to contribute to the costs of your care. There are many ways to pay for care and support. We'll talk to you about your options after the financial assessment.
