How we assess your needs and eligibility

To get the most out of the assessment, you may want to prepare by thinking about the things you want to talk about.

The assessment will usually be face-to-face. You can have someone with you, like a family member, friend or advocate.

We will ask you for your permission to record your information on our social care system. We may need to share or collect information from other organisations, including your GP. We will keep this information confidential.

We will consider short term support that can help you to maintain your independence, or stop you from needing longer term care. If you need longer term support, we will talk with you about your ongoing needs.

What will you be asked about at the assessment?

During the assessment we will talk with you about:

  • any disabilities, health needs or impairments you have
  • how these impact your ability to live independently
  • how these impact your physical, mental, social and emotional wellbeing
  • your strengths and the support you have in place already from friends, family or other services
  • what outcomes you would like to achieve and what support you may need

It is important that you tell us about all the difficulties you have. If you don’t then you may not receive the right advice, information or support.

What we will assess:

  • Your day-to-day activities.
  • Your personal care.
  • Your mobility – getting out and about, if you’ve had any falls.
  • Your home and living situation.
  • Your physical health and wellbeing.
  • Your emotional wellbeing and mental health.
  • Your relationships and social activities.
  • Your involvement in work or education.
  • Your use of facilities or services in the local community.
  • Your caring responsibilities for others, including children.
  • Your safety and any risks to it.
  • Your current support from family, friends and professional services.

If you need help during the assessment

  • We can provide you with an interpreter if you have difficulty communicating.
  • You can have someone with you during the assessment, like a family member, friend or carer.
  • We can provide you with an advocate. 
  • We will explain why we are assessing you and answer any questions you have.
  • If English is not your first language, we can arrange for an interpreter to be there at the visit.

After your assessment

We will decide if you are eligible for support based on the eligibility criteria in the Care Act 2014:

  • Are your needs caused by a physical or mental impairment or illness?
  • Because of your needs, is there anything you struggle to do in your daily life, with or without help?
  • If so, is there or is there likely to be a significant impact on your wellbeing?

We will let you know about the decision we have made and the reasons why in writing.
